& exception dates for scheduling. ie: more than one

school dances, SAT, ACT, Coach work schedule, etc. |

Referee Evaluation Form
Use
this form to evaluate how a referee performed during
your game. |

Background Check
If you intend on
registering as a coach or team official for the upcoming
2010/2011 seasonal year you will be required to complete
a background check before you can be added to a team.
Once you have completed this process and have been
cleared by Nevada Youth Soccer Association you will be
able to be placed on the team of your choice.
All coaches and team officials will be required to
complete this process. Prior clearances will not be
carried over. All clearances currently submitted will
be valid for 12 months.
